This prototype is for fictional information. Form answers are sent to the rendering server so it can validate the current step and return the next HTML page. They are not submitted to a clinician or stored in a patient database.

Preview state is encrypted and authenticated in the form page, bound to an HttpOnly session cookie, and expires after 30 minutes. The server does not log form answers. Leaving through the intake’s exit link clears the session cookie.

Cloudflare or Firebase Hosting and the server provider may process technical request information. Typography loads from Google Fonts. There are no advertising pixels or analytics scripts.

A clinical privacy policy and approved patient-data workflow are needed before accepting real medical information.
